Hazard Identification & Reporting Policy

Purpose

To create a proactive safety culture where all workers identify and report hazards before they cause harm.

Scope

All workers, contractors, and visitors at [YOUR BUSINESS NAME].

What is a Hazard?

A hazard is anything that has the potential to cause harm — injury, illness, or property damage. Examples include:

  • Spills on the floor (slip hazard)
  • Damaged racking (falling objects hazard)
  • Forklift in pedestrian area (collision hazard)
  • Frayed electrical cord (electrical hazard)
  • Excessive noise (hearing damage hazard)
  • Extreme heat (heat stress hazard)
  • How to Report a Hazard

    Option 1 — Immediate verbal report: Tell your supervisor straight away. Option 2 — Hazard Report Form: Complete FORM-001 (paper or digital via this portal). Option 3 — "Report a Hazard" button: Use the quick-access button on the portal homepage.

    What Happens After You Report

    1. Your supervisor will acknowledge the report within 24 hours.
    2. The hazard will be assessed and a risk rating assigned.
    3. Control measures will be implemented.
    4. The hazard will be recorded in the Hazard Register (REG-001).
    5. You will be informed of the outcome.

    Your Right to Refuse Unsafe Work

    Under the WHS Act 2011 (Qld), you have the right to cease or refuse to carry out work if you have a reasonable concern that it would expose you to a serious risk to your health or safety from an immediate or imminent hazard.

    If you stop work due to a safety concern:

    1. Notify your supervisor immediately.
    2. Remain available to assist with resolving the issue.
    3. You will not be disadvantaged for exercising this right in good faith.
